教案和教學設計都是事先設想的教學思路，是對准備實施的教學措施的簡要說明；教學案例則回是對已經發生答的教學過程的反映。一個寫在教之前，一個寫在教之後；一個是預期，一個是結果。
案例與教學實錄的體例比較接近，它們都是對教學情景的描述，但教學實錄是有聞必錄，而案例則是有所選擇的。

教學案例的一般要素
1．背景
所謂背景，即是向讀者交待清楚："故事"發生的時間、地點、人物、事情的起因等。背景介紹也不必面面俱到，重要的是說明"故事"的發生是否有什麼特別的原因和條件。背景是案例很重要的環節，描述的是事件的大致場景，是提供給讀者了解「事件」有用的背景資料，如所在學校的情況、個人的工作背景、事件發生的起因等。
2．主題
每篇案例要有一個鮮明的主題，即這個案例要說明的某個問題，是反映對某個新理念的認識、理解和實踐，還是說明教師角色如何轉變，教的方式、學的方式怎樣變化，或是介紹對新教材重點、難點的把握和處理，等等。
3．細節
有了主題，就要對原始材料進行篩選，有針對性地選擇最能反映主題的特定的內容，把關鍵性的細節寫清楚。要特別注意提示人物的心理。因為人物的行為是故事的表面現象，人物的心理則是故事發展的內在依據。面對同一個情景，不同的教師可能有不同的處理方式。為什麼會有各種不同的做法？這些教學行為的內在邏輯是什麼？執教者是怎麼想的？揭示這些，能讓讀者既知其然又知其所以然。在這個環節中，要講明問題是如何發生的，問題是什麼，問題可以和事實材料交織在一起。這是整個案例的主體，要詳盡地描述，展現問題解決的過程、步驟以及問題解決中出現的反復挫折，也可以涉及問題初步解決成效的描述。
4．結果
案例不僅要說明教學的思路，描述教學的過程，還要交待教學的結果--某種教學措施的即時效果，包括學生的反應和教師的感受，解決了哪些問題，未解決哪些問題，有何遺憾、打算、設想等。以「問題」為主線，有矛盾、沖突甚至「懸念」，能引起讀者興趣和深入思考。
