英語請假條格式如下：
標題：居中寫《請假條》，用來表明此文是用來請假的。
稱呼：平時怎麼稱呼，這里就怎麼寫，後面加冒號。這個地方往往是領導、相關負責人等。

請假條與書信、書面通知、啟事、海報、日記、留言條、履歷表等一樣，是應用文之一，是請求領導或老師，准假不參加某項工作、學習、活動的文書。請假條分為病假條和事假條，無論是請事假還是請病假都要注意在正文中把請假原因、時間等寫清楚，請假理由要充分。若是病假最好附上醫生證明。
