「請假申請」相當於公文中的「請示」，但比請示簡便、靈活，格式可以不固定， 也可以固定。一般來說，請假條應該包括以下方而的內容：

關於請假申請怎麼寫的幾點寫作提示：
釆用書信格式寫的請假條，日期應寫在正文的右上角。由於請假條的內容太多， 可能是當天、近幾日因病或因事請假，所以不必寫上年份；稱呼寫在左上角。
請假條如是向上級請示，一般以「Dear 」或「My dear ...」開頭。有時可在「Dear」之後，用 上「Mr」或「Miss」等的稱呼加上姓氏。不過，有一點仍要提醒大家，各種稱呼後而 一般都用逗號。正文要力求簡潔明了，用詞口語化。
寫請假條只要把請假的理由和 請假時間說清楚即可。有的請假條為了說明其請假事由是真實的，在正文之後另起一行，寫上「I enclose a doctor』s certificate.」（我附上了醫生證明。）簽名(署名)寫在 右下角，其上面一行可以寫上「Yours」或「Your student"等字樣，有時由於寫條的人 和收條的人很熟悉，所以在稱呼和簽名上比較隨便和簡單。特別是在簽名部分，有 時可簡單到只寫個姓或只寫個名。
