我不知道你這個是網上提交推薦信還是用紙質推薦信推送。網申的話，只要留下版老師的權郵件，然後按收到郵件信息上傳到一個推薦信系統里就可以。紙推的話，將推薦信列印出來由老師簽名，並在信封封口處也要簽名以確保保密信。
格式如下：
日期，
【換行】
老師個人信息（包括姓名，職務，工作地點，聯系方式）
【換行】
正文 （正文稱謂為"To whom it may concern"）

用學校的信紙肯定是最好了。更正式，如果是在沒有也沒有關系，關版鍵要有老師的簽名，聯系電權話和郵箱。
成為就直接說English teacher就可以了，不用強調高級，如果是班主任的話在加上head teacher的頭銜，關鍵不是推薦老師有多麼優秀，關鍵是推薦人有多了解你。
