我不知道你这个是网上提交推荐信还是用纸质推荐信推送。网申的话，只要留下版老师的权邮件，然后按收到邮件信息上传到一个推荐信系统里就可以。纸推的话，将推荐信打印出来由老师签名，并在信封封口处也要签名以确保保密信。

格式如下：

老师个人信息（包括姓名，职务，工作地点，联系方式）

用学校的信纸肯定是最好了。更正式，如果是在没有也没有关系，关版键要有老师的签名，联系电权话和邮箱。

成为就直接说English teacher就可以了，不用强调高级，如果是班主任的话在加上head teacher的头衔，关键不是推荐老师有多么优秀，关键是推荐人有多了解你。
